I disagree with a medical note or comment that my doctor put in my medical file. How do I contest it?
California Health and Safety Code Section 123111 provides that a patient may submit a written addendum to any statement or item in his or her medical record that the patient believes to be incorrect or incomplete. The statement may not exceed 250 words per alleged incorrect or incomplete item in the record and must clearly state that the patient would like the statement to be part of his or her medical records. The health care provider must attach the addendum to the patient’s medical records and must include that addendum whenever the provider discloses the allegedly incorrect or incomplete portion of the records to a third party.
